In this learning activity, you will add a SOAP note to the chart of Justin Monroe, a 12 year old patient with a sore throat, ear pain, and nasal discharge. There are times, when interacting with a patient, that the interview and exam don’t go along in the order the book says they should. This encounter is an example of one of those times. The practitioner, Kate Walsh, has completed her interview and physical exam.

These buttons will remain available as you work through each section. In addition, guidance about how to write each part of the SOAP note is provided within each section.
